Admissions Procedure

Reach out to us by phone, email, or in person to express your interest. We’ll answer any questions you may have and provide basic information about our services, availability, and eligibility. 

We will assist you in connecting with your doctor or case manager to obtain a referral for adult day care services. 

Once your referral is received, you or your loved one will undergo an assessment to determine individual needs and ensure proper care planning. 

You may be asked to provide basic medical records or complete any additional forms required by your provider or our facility. 

Once approved, our team will receive all necessary documents from your case manager. We’ll review and finalize the paperwork, then schedule a start date for services. 

If you are already approved for Adult Day Care Services, you don’t need to go through all of these steps—just contact us, and we will enroll you or your loved one in our program. If you do need to go through the full process, don’t worry—we are here to guide and support you every step of the way.
