As part of the admissions procedure, you may be asked to provide basic medical records or complete any additional forms required by your provider or our facility, especially if you are seeking adult day care services or need referral support.

Once the admissions procedure is approved, our team will receive all necessary documents from your case manager. We’ll review and finalize the paperwork for the adult day care services, then schedule a start date for services, ensuring you have the referral support you need.

If you are already approved for adult day care services, you don’t need to go through the admissions procedure—just contact us, and we will enroll you or your loved one in our program. If you do need to complete the full admissions procedure, don’t worry—we are here to provide referral support and guide you every step of the way.
